Request for quotation and purchase order and contract forms

Effective December 2015, updated December 2023

Request for quotation (Form AC565) and purchase order and contract (Form SP020) forms should be used when undertaking procurements which are for firm price, commercially available goods and repair services, where the risk to the Commonwealth is low and where the time frame for delivery is short.

Further guidance on when to use the AC565 and SP020 forms has been included in the Contract template selection and tailoring guide.

The AC565 form was updated in December 2023, incorporating the following changes:

  • Requirement to notify country of tax residency
  • Removal of fax numbers and notice by facsimile, and alignment of notices clause to clarify when notices are received.
