This form is used to report changes to a previously submitted Foreign Work Authorisation (FWA). An individual with an FWA must report changes that occur during the term of the FWA. Reportable changes include:
- the type of work or training being performed
- the location where the work or training is being performed
- the organisations or bodies which may benefit from the work or training
- contact details (address, phone number, email)
- start date, end date or duration of the work or training (3 year maximum from grant date)
- alteration to any FWA conditions.
Some changes require a new FWA request to be submitted to replace a current FWA, including:
- changes to the employer
- the duties of work or training being performed.
