Foreign work authorisation variation

This form is approved by the Secretary of the Department of Defence under s 115H(2)(b) of the Defence Act 1903 (Cth).

Giving false or misleading information is a serious offence.

This form is used to report changes to a previously submitted Foreign Work Authorisation (FWA). An individual with an FWA must report changes that occur during the term of the FWA. Reportable changes include: 

  • the type of work or training being performed
  • the location where the work or training is being performed
  • the organisations or bodies which may benefit from the work or training
  • contact details (address, phone number, email)
  • start date, end date or duration of the work or training (3 year maximum from grant date)
  • alteration to any FWA conditions.

Some changes require a new FWA request to be submitted to replace a current FWA, including:

  • changes to the employer
  • the duties of work or training being performed.
