How to Register

Before completing your AD858-1 ADF Family Health form, have you:

  • read the Conditions Of Use?
  • ensured that your dependant details are current and accurate in the Dependant/Beneificary Section of PMKeyS (ie Name, Address and Date of Birth)?  If dependant details are incorrect or incomplete:
    • submit an AD160 Dependant Details to your Admin Officer or Customer Service Centre
    • Confirm your PMKeyS Dependant Details have been updated correctly

You should then:

  • Complete the Form AD858-1 ADF Family Health Form, ensuring information is consistent with PMKeyS.
  • Ensure each Dependant Registration has the appropriate Dependant consent provided.
  • Fax the form to the ADF Family Health Directorate: (02) 6266 4292; or

Upon successful registration, you will be provided with an ADF Family Health Card which you will use to access the Allied Health benefits associated with the Program/Trial. Participation in the trial does not commence until the card has been received.

Deployment and Registering Dependants
